“He went into cardiac shock in the morning and passed away during treatment. His reports were normal, but symptoms pointed to Dengue leading to drop in platelets count to 18,000,” a doctor from the hospital told TOI.

Dr Rajeev Gehlot was an expert in treating complex congenital heart conditions and various adult cardiac surgeries. He had vast experience in doing over 5000 cardiovascular & thoracic surgeries in the last 12 years at different centres across India.

SHRC Directs DMO to inspect Valiathura Coastal Hospital

Thiruvananthapuram: The State Human Rights Commission (SHRC) has directed the District Medical Officer (DMO) to assess the infrastructure at Coastal Speciality Hospital in Valiathura, to determine its readiness to resume inpatient and 24/7 services. The inspection aims to address concerns about the hospital’s capacity to provide round-the-clock medical care, which was halted during the COVID-19 pandemic.

Commission chairperson Alexander Thomas emphasized the importance of understanding the specific infrastructure needs for restarting services. The DMO has been asked to provide a detailed report within six months, which should include details such as the number of doctors, nurses, and other staff needed at the hospital; the current staff strength; the number of beds in the hospital; and whether it has the infrastructure required to restart inpatient treatment.

Before the pandemic, Coastal Speciality Hospital offered both inpatient and 24-hour services. However, due to a lack of adequate infrastructure and resources, these services were suspended. A report submitted by the DMO noted that the hospital currently employs six doctors, but at least three more are needed to resume inpatient care. It also highlighted that inpatient services could be restored if the structural infirmity and seepage in the hospital building were addressed.

As per the recent media report by The Hindu, the city Corporation Secretary told the commission that according to a circular issued by the Local Self-government department on October 25, 2023, there was permission to appoint only one doctor and one paramedical staff member in the hospital under the Corporation.

The SHRC’s intervention follows a petition filed by Valiathura resident Jerome Meeran, who raised concerns over the lack of medical services in the area.

Mizoram Approves Plan for Neurosurgery Dept at Zoram Medical College

Mizoram- Mizoram health authorities recently approved the detailed project report (DPR) for the proposed setting up of a neurosurgery department at the only Zoram Medical College (ZMC) in Falcon near Aizawl.

On December 2, a meeting of Mizoram health officials was held at Raj Bhavan, which was chaired by the state governor Hari Babu Kambhampati. In the meeting, the urgent need to establish a Department of Neurosurgery in ZMC to meet the health demands of the people of Mizoram was thoroughly discussed.

The meeting was attended by the State Health and Family Welfare Minister Lalrinpuii, Chief Minister’s Advisor (Finance and Planning) TBC Lalvenchunga, ZMC Director Dr John Zomingthanga and neurosurgeon Dr Aaron M Pillai of Silverline Hospital, Kochi.

As per the TOI news report, the Governor emphasised the importance of creating a systematic approach to effectively utilise Corporate Social Responsibility (CSR) funds. Azithromycin Effective Alternative to Clarithromycin in H. pylori Therapy, suggests research

Researchers have found that azithromycin is a good substitute for clarithromycin in the combination therapies of triple and quadruple therapy for the eradication of Helicobacter pylori. This conclusion is reached through the study of a retrospective report carried out at King Hussein Medical Centre, Amman, Jordan. The study was published in the Cureus by Sami H. and colleagues.

This was an observational, single-center study involving 1,076 patients treated for H. pylori infection at King Hussein Medical Centre from January 2023 to May 2024. Patients were stratified into six eradication regimens (I-VI) based on the macrolides used: clarithromycin or azithromycin, PPI doses, and treatment duration, which were either 7 days or 10-14 days. Clarithromycin was used in Regimens I-III, and azithromycin was used in Regimens IV-VI.

Proton Pump Inhibitor (PPI) Doses:

• Low-intensity dose: 20-40 mg/d (omeprazole).

• High-intensity dose: 80 mg/d.

Patients were submitted to a stool antigen test, six weeks after treatment, for verification of eradication. PPIs were withdrawn two weeks before the test to avoid biases. Treatment adherence and the statistical association of treatment success with variables such as macrolide type, length of regimen, and degree of adherence were assessed with chi-square tests and through multiple logistic regression models.

Patient Characteristics:

• Number of patients: 1,076.

• Gender: 49.3% female (530 patients), 50.7% male (546 patients).

• Age: 78.2% (841) less than 60 years; 21.8% (235) 60 or older.

Eradication Success:

• Significantly higher eradication rates for azithromycin-based treatments than for clarithromycin-based treatments.

• Odds ratio of azithromycin treatment effect on success: 1.780 (95% CI: 1.378-2.299).

Treatment Adherence:

• Patient adherence had a significant effect on treatment success. Compliance plays a significant role in H. pylori eradication therapy.

• Most of the treatments lasted 7-14 days, but there were no significant differences in terms of effectiveness when adherence was kept.

Azithromycin would be a reasonable and effective alternative to clarithromycin for H. pylori infection, especially in triple and quadruple eradication regimens. Optimizing macrolide selection, treatment duration, and adherence strategies will improve the rate of eradication and better outcomes for patients, especially those residing in areas with limited antibiotic choices.
