Jaslok Hospital Doctors perform Bilateral Micro Vascular Decompression on 56-year-old woman suffering from bilateral facial pain

A 56-year-old housewife from the interiors of Maharashtra, suffering from this debilitating condition, has experienced complete pain relief following Bilateral Micro Vascular Decompression (MVD), marking a major advancement in neurosurgery and the first in India.

The patient, with a five-year history of severe, shock-like pain on both sides of her face, endured excruciating episodes that lasted several minutes and were triggered by everyday activities such as talking, eating, brushing her teeth, and even exposure to a cool breeze. Despite various medical treatments, she found no relief because the disease was not diagnosed. The excruciating pain made it difficult to perform basic household duties to an extent that it led to the development of suicidal thoughts.

Referred to Jaslok Hospital in October 2023, Dr Raghvendra Ramdasi diagnosed her with Bilateral Primary Trigeminal Neuralgia. An MRI scan revealed vascular loops compressing her trigeminal nerves (fifth cranial nerve) on both sides.

Primary Bilateral Trigeminal Neuralgia is an extremely rare, with an incidence ranging from 0.6% to 5.9%. To date, only about 24 cases have been reported in the English medical literature, with none from India. This case marks the first reported instance in India where Bilateral MVD was performed, resulting in complete pain relief.

The trigeminal nerve, or fifth cranial nerve, supplies sensation to the face through three divisions. Trigeminal Neuralgia is a chronic pain disorder characterized by sudden, severe facial pain, often described as electric shock-like. Typically unilateral, the condition affects one or more divisions of the trigeminal nerve. In about 85% of cases, the cause is a vascular loop compressing the nerve at its root entry zone.

While medications such as Carbamazepine, Gabapentin, Lamotrigine, and Topiramate can offer relief, Micro Vascular Decompression (MVD) remains the gold standard treatment. This surgical procedure involves placing a Teflon sponge between the nerve and the compressing vessel.

Under the leadership of Dr Raghvendra Ramdasi, Consultant Neurosurgery at Jaslok Hospital, the medical team including Dr Poornima Shah, Consultant Neurophysiology, Dr Rajani Prajish, Consultant Anaesthesiology, Dr Savi Kapila, Consultant Anaesthesiology performed Bilateral Micro Vascular Decompression, initially operating on the more severe left side, followed by the right side a week later. Post-surgery, the patient experienced complete pain relief, significantly improving her quality of life. Six months later, she continues to live pain-free, able to resume normal activities without fear of pain attacks.

MCC to conduct NEET MDS Counselling of Priority- III, IV, V candidates into AFMS PG Teaching Institutes from this year, all details here

New Delhi- The Medical Counselling Committee (MCC) has informed through a notice that from this year, counselling for Priority III, IV and V candidates in Armed Forces Medical Services (AFMS) PG teaching institutes will be conducted by the MCC of DGHS. In addition, FAQs for candidates desirous of participating in AFMS MDS counselling 2024 have also been released.

AFMS conducts postgraduate training in AFMS PG Teaching Institutes primarily for the in-service medical and dental officers of the AFMS to fulfil the specialist requirements. Surplus seats, if any, are offered to other eligible candidates as per priority system sanctioned by the Govt of India. The civilian doctors who get admission in postgraduate courses (MD/MS/DNB/MDS) at any AFMS institution shall be liable to serve as Short Service Commissioned Officers (SSC) for 05 years in AFMS.

As sanctioned by the Govt of India, after fulfilling the AFMS requirements, the remaining seats in the AFMS PG Teaching Institutes, if any, are made available to other eligible candidates as per the priority system outlined below-

1 Priority-I: AFMS Officers detailed on Advance Specialist Course/ PG courses.

2 Priority-II: Foreign students sponsored by Government and AFMS officers granted Study Leave.

3 Priority-III: Medical Officers sponsored by Para Military Organization/ other Govt. of India organization.

4 Priority-IV: Ex-SSC AFMS Officers within 03 years of their release from service.

5 Priority-V: Civilian candidates.

As per the notice, the counselling for Priority-I & II candidates will be conducted by O/o DGAFMS and counselling of Priority-III, IV & V candidates will be conducted by MCC. Candidates of all priorities are advised to register themselves on the MCC portal for counselling as per the schedule. More information in this regard will be available on the MCC official website and the AFMS website. Seat allotment during counselling by MCC is provisional and subject to fulfilment of all eligibility criteria as mentioned in the ‘Information Bulletin for PG Courses in AFMS Institutions through NEET PG-2024 and NEET MDS-2024’. AFMS reserves the right to deny admission to any candidate who is found to be ineligible as per the eligibility criteria enumerated in the Information Bulletin mentioned. The redressal of disputes, if any, shall only be through civil courts having jurisdiction of New Delhi alone.

The last date for the sponsorship certificate of Priority-III candidates & applications of Priority-IV candidates for MDS seats in AFMS Institutions to reach the O/o DGAFMS is 05 July 2024.

28 seats in 11 specialities, NBE Releases Centralised Merit-Based Counseling Schedule for FNB FPIS courses for International Students 2023 session

New Delhi- The National Board of Medical Examinations (NBEMS) has released the schedule for offline centralised merit-based counselling for international students- FNB Fellowship Programme for International Students (FPIS) courses for the 2023 admission session.

The Centralised Merit Based Counseling for admission to FNB (FPIS) courses for the 2023 Admission Session will be conducted by NBEMS via offline mode. Participation in the offline Counseling will be strictly followed as per the eligibility criteria for the FNB- FPIS 2023 Admission Session.

The seat allocation will be done in order of merit cum choice/ preference of seats indicated by the candidate during the registration/choice submission process and the provisional seat allotment result of the eligible candidates will be published on the NBEMS website.

To join the allotted seat, the candidate is required to pay the First-year course fee, which is nonrefundable within the stipulated time. After the payment of the First-year course fee, the seat allotted to him/her will be frozen in his/her favour. The details regarding the submission of the annual course fee will be communicated to the candidate in the seat allotment letter issued by NBEMS.

Along with this, NBE has also released an indicative seat matrix for FET-FPIS 2023. As per the seat matrix, there are a total of 28 vacant seats in 11 specialities.

NEET MDS 2024: UPDGME Releases Instructions for Online Counselling candidates

Uttar Pradesh- Uttar Pradesh Director General, Medical Education & Training (UPDGME) has released certain important information/instructions regarding the online counselling of UP National Eligibility cum Entrance Test-Master of Dental Surgery (NEET MDS) 2024.

Below are the important information/instructions regarding the online counselling of UP NEET MDS 2024-

1 UPDGME will conduct the online counselling of UP NEET MDS-2024 in collaboration with the State Informatics Centre (NIC), a technical body nominated by the Government of Uttar Pradesh. Admission taken at the level of any unauthorised institution/person is completely illegal, for which the UPDGME will not be responsible.

2 All admissions taken by any unauthorised entity/individual or by self will be treated as completely illegal. If in future the office receives any complaint regarding such unauthorised entry then the entire responsibility will be of the candidate himself. On which the department will have no responsibility.

3 Only the candidates declared successful in the NEET MDS-2024 examination conducted by NBEMS are eligible to participate in the UP NEET MDS-2024 counselling process.

4 All information related to online counselling of UP NEET MDS-2024 will be displayed on the departmental official websites.

5 Candidates willing to take admission to private sector dental colleges should know about the respective colleges and the scholarships from the concerned college by themselves. Or the parents themselves should collect complete information at their own level. The Office, Government/Directorate General is not responsible for any reply or take any responsibility thereon.

6 Reservation Certificates of Other Backward Classes and Economically Weaker Sections (EWS) of Uttar Pradesh should be made on or after 01 April 2024. SC/ST/OBC/EWS category candidates are advised to collect the reservation certificate issued by the Government of Uttar Pradesh in an updated format to facilitate online verification of the certificate.

7 All certificates related to reservation (OBC, Scheduled Caste, Scheduled Tribe and Economically Weaker Section) (except Disability Certificate) issued by the competent authority of Uttar Pradesh on the format prescribed by the Government of Uttar Pradesh will be valid. However, any certificate regarding reservation not issued in the format prescribed by the Government of India will not be valid. Apart from Uttar Pradesh state, reserved category candidates from other states will be considered in the unreserved category.

AstraZeneca Imfinzi fails in late-stage trial to treat type of lung cancer

Cambridge: AstraZeneca has announced the high-level results from the ADJUVANT BR.31 Phase III trial, sponsored by the Canadian Cancer Trials Group (CCTG), showed Imfinzi (durvalumab) did not achieve statistical significance for the primary endpoint of disease-free survival (DFS) versus placebo in early-stage (IB-IIIA) non-small cell lung cancer (NSCLC) after complete tumour resection in patients whose tumours express PD-L1 on 25% or more tumour cells.

​Susan Galbraith, Executive Vice President, Oncology R&D, AstraZeneca, said, “We are disappointed in the ADJUVANT BR.31 results. Imfinzi has helped change the treatment landscape and achieved multiple positive Phase III trials for patients with earlier stages of lung cancer. We are committed to addressing the remaining unmet need in lung cancer through our broad development programme.”

The safety profile for Imfinzi was consistent with its known safety profile, and no new safety concerns were reported. The data will be shared at a forthcoming medical meeting.

Imfinzi is the only approved immunotherapy and the global standard of care in the curative-intent setting of unresectable, Stage III NSCLC in patients whose disease has not progressed after chemoradiotherapy based on the PACIFIC Phase III trial.

Imfinzi is also being investigated as monotherapy and in combinations in several other early-stage lung cancer settings, including in medically inoperable or unresected Stage I-II NSCLC (PACIFIC-4) and unresectable, Stage III NSCLC (PACIFIC-5, 8 and 9).

Each year, there are an estimated 2.4 million people diagnosed with lung cancer globally. Lung cancer is the leading cause of cancer death among both men and women, accounting for about one-fifth of all cancer deaths. Lung cancer is broadly split into NSCLC and small cell lung cancer (SCLC), with 80-85% of patients diagnosed with NSCLC.

The majority of NSCLC patients are diagnosed with advanced disease while approximately 25-30% present with resectable disease at diagnosis. Early-stage lung cancer diagnoses are often only made when the cancer is found on imaging for an unrelated condition.

The majority of patients with resectable disease eventually develop recurrence despite complete tumour resection and adjuvant chemotherapy. Only around 58% of patients with Stage IB disease will survive for five years. This decreases to 36-46% for patients with Stage II and 24% for patients with Stage IIIA disease, reflecting a high unmet medical need.
